Q12: What is a Doctor's Order?
A: A Doctor’s Order or Physician’s Order Form is a special form, similar to a prescription, that your doctor must complete, sign, date and return to us. The form lists the diabetic testing supplies your doctor recommends you use. In addition, it contains necessary information, such as your diagnosis code and the number of times your doctor would like you to monitor your blood glucose level daily. G & H is a Durable Medical Equipment supplier. We bill under the medical benefits section of Medicare, Texas Medicaid, and select private insurance plans. These insurance plans require that we have a valid Doctor’s Order on file noting that you are eligible to receive diabetic testing supplies. With your permission, we will contact your doctor on your behalf, and secure the appropriate form.

Q13: Are there limits of the number of supplies that will be reimbursed?
A: Medicare does have established limits based on whether you are treated with insulin or not. Your physician will provide G & H with a Physician’s Order Form that will indicate how often they recommend you test. Medicare may approve additional diabetic testing supplies if your physician determines it is medically necessary to control your diabetes.
